TERMS OF AGREEMENT
Upon receipt of this agreement, please review the details then sign the agreement and use the Agree & Sign button below.
Your deposit can be paid online Via Credit Card, PayPal or Venmo Payment. We must receive your confirmation and deposit within 7 business days of the date of this letter in order to secure your booking. Otherwise, your date may be released from our calendar.

Please print and mail a copy of the agreement with your nonrefundable 25% deposit check of the stated compensation along with a signed copy of this agreement to:

It is understood by both parties that the balance of $00.00 (less the 25% deposit) will be paid in full upon arrival or immediately following the performance on .

In the event that the performance must be canceled on behalf of the Presenter or Artist due to circumstances beyond their control, the Presenter and Artist shall be respectively relieved of their obligations with respect to this performance. In the case of a cancellation by the artist, the 25% deposit will be refunded.

I should arrive approximately 45-60 minutes early (prior to time show time) to set up equipment.
Private early set up, in a spacious room is helpful.

NOTE: Due to time limitations and performance schedule (sometimes I have multiple performances in a day), the program must begin promptly and as close to the agreed upon time as possible. Thank you in advance for your understanding and consideration.

Taking flash pictures of the performance is fine. Videotaping is prohibited during performances unless granted prior consent.

Please sign this agreement within 7 days. If this agreement and deposit fee of $00.00 isn't returned promptly, your date may be released. By signing this agreement, the sponsor agrees to invest in a Jason Purdy performance. A 25% non-refundable booking fee is required and due upon completion of signing this agreement. Should the engagement for which the contract is made be canceled by the sponsor more than two weeks prior to the engagement, without cause from an act of god, the sponsor shall forfeit any and all booking fees. However, if the sponsor decides to reschedule the engagement, any booking fees made can be applied to rescheduled performance within one year after the original event date. Please note: that any rescheduled event is subject to availability. If sponsor cancels the engagement for any reason whatsoever within two weeks of the performance, the remaining balance must be paid in full and payable to Jason Purdy within two weeks of cancellation. If for any reason Jason Purdy has to cancel the event, another competent entertainer will be found or the booking fee will be repaid in full. Due to the busy performance schedule, all performances must begin on time. Thank you!
